Human milk composition is not static; it is an evolving fluid that responds to the gestational age of the infant at birth. Mothers who deliver prematurely produce milk that is different from that of mothers who deliver at term.
Preterm milk is significantly higher in protein and total nitrogen compared to milk produced for term infants. This elevation is most pronounced during the first few weeks of lactation, known as the colostral and transitional phases.
The protein fraction consists of casein and whey. The whey fraction is particularly rich in bioactive molecules like secretory immunoglobulin A (sIgA), lactoferrin, and lysozyme. These proteins do not just provide amino acids. They actively protect the infant against pathogens.
Preterm milk also contains higher levels of non-protein nitrogen. This includes urea, nucleotides, and amino acids that assist in cell signaling and DNA repair.
It also contains higher levels of docosahexaenoic acid (DHA) and arachidonic acid (ARA) compared to term milk. These fatty acids are the primary structural components of the brain and retina.
Preterm milk contains higher levels of sodium, chloride, magnesium, and iron compared to term milk. These minerals are essential for maintaining blood pressure and supporting enzyme functions in a developing body.

Protection Against Necrotizing Enterocolitis (NEC)
Preterm infants are at risk of necrotizing enterocolitis (NEC). It’s a severe intestinal condition where intense inflammation and bacterial infection lead to the death of tissue within the bowel wall.
Mortality rates of NEC are as high as 30%. According to WebMD, the risk of NEC is the highest in babies who are formula-fed instead of breast milk in the first 14 days after birth. Cow milk-based formulas of Similac and Enfamil are linked to NEC in preemies.
Not surprisingly, lawsuits are being filed against formula manufacturers in both state and federal courts. The major defendants in the NEC baby formula lawsuit are Abbott Laboratories and Mead Johnson & Company, the manufacturers of Similac and Enfamil, respectively.
TorHoerman Law notes that the lawsuits allege that manufacturers failed to inform parents and medical professionals about the heightened risks of using their formulas.
Feeding a preterm infant breast milk is the best way to prevent this problem. The underdeveloped intestines of infants are prone to severe inflammation because they have high levels of the protein Toll-Like Receptor 4 (TLR4), which overreacts to bacteria.
However, human milk contains growth factors (EGF) that bind to specific receptors in the gut. This process shuts down the inflammatory TLR4 pathway, helping to prevent the tissue damage seen in NEC.
Better Brain Development and Improved Cognitive Function
The neurological development of a preterm infant is highly sensitive to early nutritional inputs.
Infants born 10 weeks or more early (average 26 weeks of gestation) have brains that are not yet fully developed and are significantly smaller than those of full-term infants. Breast milk promotes structural brain growth in ways that formula cannot replicate.
Research published in MJ Nutrition, Prevention & Health confirms this. It found that premature babies who were fed 50% of breast milk during their first month had more grey matter volume at term-equivalent age. They also had better IQ and performed better on school tests, memory tasks, and physical coordination.
The development of white matter, the brain’s communication network, is highly dependent on the intake of specific fats found in human milk.
Long-chain polyunsaturated fatty acids (LC-PUFAs), specifically DHA and arachidonic acid (AA), are essential components of neural cell membranes and the myelin sheath. Myelin acts as insulation for nerve fibers, allowing for the rapid and synchronized transmission of electrical signals.
What’s more, breastfeeding also reduces the risk of minor neurological dysfunctions and neurodevelopmental impairment.
